BioForm Medical,which focuses on developing and commercializing products that are used by physicians to enhance a patient’s appearance, filed for an IPO Monday with the SEC. The company filed to be listed on the NASDAQ under the ticker "BFRM." JP Morgan and Piper Jaffray are the lead managers on the deal. No terms were disclosed in the initial filing.
